LaTeX:

- Replace " with \texttt{"}, which is ugly, but less ugly than quotes going
in the wrong direction.

- Add a disclaimer to the front of the book telling people to look
at the html if they have a problem.

- Use "longtable" (and thereby allow a pagebreak) for tables with
more than 15 rows.
