Proxy module moved to modules/proxy. Its dependencies in Makefile.tmpl are

now handled in its own Makefile. This sets up to be out of the way of future
main source dir changes, and reorganize the proxy, splitting the massive
source file, fixing things, and moving towards a real protocol handler
abstraction, among other things.
