Fixing two problems:

1. Register a cleanup for the socket (missed the call to ap_note_cleanup... in previous version)
2. Signal the parent to die on error. This will prevent the parent from attempting to restart the child.
Both of these fixes are already in the Apache 2.0 winnt mpm.
