* modules/ssl/ssl_engine_pphrase.c (ssl_pphrase_Handle): Simplify to

  calculate hash index only once per loop iteration; no (intended)
  functional change.  Add comments and tweak a log message.

* modules/ssl/ssl_private.h: Add comments.
